The chief inspector of prisions says that inmates in England and Wales who potentially pose a "high risk of harm" are being freed "without a full risk assessment".

Peter Clarke says too many jails are plagued by violence, drugs and "appalling living conditions".

The government insists there have been improvements in risk assessments, and 70 million pounds has been spent on improving safety and decency.
